All Covered California members (except those in minimum coverage plans) should have received the Federal Internal Revenue Service (IRS) Form 1095-A Health Insurance Marketplace Statement. If you had a minimum coverage plan, you may receive a 1095-B tax form from your health insurance company, but the form is not required to file your taxes. This document is used when you file returns to:

  • Calculate your tax refund/credit or the tax amount you owe if you received financial help in 2022. At tax time, the IRS compares any financial help you received during the year based on the estimated income information you reported on your application versus what you truly qualified for based on the income information on your tax return. This is known as “reconciliation.”

  • Show that you were insured so that you don’t pay a penalty for the months you were covered.

ATTENTION: Please note, you will not receive a Form FTB 3895 for the 2022 tax year.

If you have not received your tax document yet, please contact the Covered California Service Center at 800-989-2199, Monday through Friday, 8 a.m. to 6 p.m. for assistance.

Thank you for being a Covered California member!
